Certified Skilled Nursing Care

Our highly trained and qualified Registered and Licensed Practical Nurses come to your residence to provide all facets of nursing care and are available 24/7 in Passaic, Bergen, and parts of Morris counties. Our nurses follow your physician’s plan of care, and provide reports and updates to your physician(s) as needed to monitor progress. Our team will also provide updates to family and loved ones, if helpful, to explain the care program and educate them so the entire family and “circle of caregivers” are working together to support the patient’s physical and emotional needs.

Our skilled nursing care services include:

• In-home assessments and evaluation to establish a plan of treatment at home after discharge from hospital, care facility, accident, illness, or change in condition
• Care of surgical wounds, dressings, feeding tubes, as well as care and instruction of Foley catheters and drains
• Specialized home orthopedic and joint replacement, stroke, brain injury, bariatric and other condition-specific recovery programs available
• Pain control & symptom management
• Instruction and monitoring of care and progress
• Licensed Medical Social Workers
• Certified home health aides as medically needed
• Medication review, education, and organization
• Nutritional counseling
• Telehealth - remote monitoring

We also offer assistance in understanding Medicare and private insurance reimbursement services to help verify and coordinate benefits.
